Dear Christ the King Parents,

In order to participate in physical education classes or to play any sport under the Christ the King School name, the information requested below must be submitted. Every student who participates must have this form on file in the school office.

At the present time, our Parent-Student Handbook states that this is a requirement. It is no longer a requirement, but a recommendation that your child have a physical form on file. Our school does not provide for physicals, so it is your responsibility to choose to provide the school with a completed physical form. 

I request that my child(ren) be allowed to participate in Christ the King P.E. class and/or school athletics. My child(ren) is/are in good health and able to participate. 

I understand that it is my responsibility to choose to provide the school with a completed physical form for each child. I agree to hold Christ the King and all of its employees harmless on any claim due to an accident or injury from any P.E. class and/or athletic participation.
